Pathway mapping can be described as a process of creating a visual representation of the steps involved in diagnosis and treatment of a patient. The goal is to identify the blocks to an optimal care pathway for patients highlighting, in a mapped diagram, where any team could examine the efficiency and effectiveness of care delivery.
To help you map your pathway, two Amgen Healthcare Solutions Managers will interview individual specialist healthcare professionals including physicians, nurses, and other relevant team members.
These steps describe the activity.
Step 1. Identify the key points in the patient journey, including screening, diagnosis, staging, treatment options, and follow-up care. This is done by setting up one to one or group interviews with key clinical and non-clinical professionals who are accessed at various points on the patient’s journey.
Step 2. Together we outline the standard procedures, tests, and assessments involved in each step, looking at current practices and identifying areas for improvement.
Step 3. A visual representation using a VISIO diagram is created of the pathway, highlighting key decision points, time frames, and how care is coordinated. A final workshop with the Team is the last step and Healthcare Solutions Managers will present back the final visual pathway.

Pathway mapping is offered after initial conversations around the value of looking more deeply at specific services that the NHS currently provides to patients. This can be across various therapy areas including cardiovascular, inflammation, and oncology.

Discussion of the issues and solutions can take place to help develop an action plan after the potential solutions have been rated and assessed for feasibility.
The Clinical Team can take forward an action plan they have created and assess implementing the pathway in clinical practice, by tracking and evaluating its impact on patient outcomes and the efficiency of care delivery.
The clinical pathway mapping exercise is a tool examining the quality of care for patients. This can affect patient outcomes, wait time, and the use of healthcare resources.
